The 4th Scandinavian Forum for Travel Medicine 2004 will take place at Stockholm City Conference Centre, Folkets Hus, between May 9–11, 2004. Address: Barnhusgatan 12–14. For further information please visit the conference venue website: www.stoccc.se.

Advance Registration

If you have access to the Internet, please use the secure on-line registration form.

If no internet access is available, please complete and return this registration form (downloadable pdf), to Stockholm Convention Bureau, StoCon i.e. the Conference Secretariat.

The registration form is also to be used for hotel reservations as well as social events.Your registration will be taken care of on a “first come – first served” basis. Events included in the registration fee also need to be pre-booked.Tours, events and hotel reservations will be confirmed only when requested payment and/or credit card guarantee has been received.

Registration and fees

Due to Swedish tax legislation, delegates from different countries should pay the registration fee either including or excluding Swedish VAT. Would you, therefore, please identify your category below and choose the appropriate alternative on the registration form:

1. All delegate fees paid from countries outside the European Union: Excl VAT
2. Delegate fees paid by companies and other taxable entities within the European Union: Excl VAT
3. Delegate fees paid by non-taxable entities (institutions/ organisations etc.) and private persons within the European Union: Incl VAT
4. All delegate fees paid within Sweden: Incl VAT

Registration fee covers:

Participation in the sessions, Morning and afternoon Coffees, Get-together reception, Lunch Reception at the Stockholm City Hall and Conference Dinner.

Accompanying person’s fee covers:

Participation in the Get-together reception, Lunch Reception at the Stockholm City Hall and Conference Dinner.

Cancellation of your participation

Cancellations have to be in writing and sent to StoCon.The cancellation policy is the following: For cancellations received before April 5, 2004 there will be a cancellation charge of SEK 500. Thereafter a cancellation fee of SEK 1000 will be charged. For cancellation of hotel accommodation, special cancellation rules apply; please see Accommodation for details.

Processing and publication of personal data

Due to common rules adopted within the EU and Sweden, personal data of the “The 4th Scandinavian Forum for Travel Medicine 2004” participants may be used even after the meeting has been held (to render an account of the conclusions arrived at during the meeting, for statistical purposes, and for the dispatch of information on future meeting within the field in question). We require notification of your consent to this data processing by your signature on the registration form.

Payment

All payment should be made in Swedish Kronor (SEK) and made payable to Stockholm Convention Bureau, “The 4th Scandinavian Forum for Travel Medicine 2004”, P.O. Box 6911, SE-102 39 Stockholm, Sweden.
Please ensure that the name and Company/Hospital for each participant is printed clearly to ensure that the payment will be correctly registered. Payment should be submitted at the same time as the registration form by one of the following means:

1. Credit cards accepted: American Express, Diners Club, Master Card, Euro card and Visa. These may be used for all the costs on the registration form. The card number and expiry date should be stated in the appropriate space on the registration form.
2. Bankers’ draft or cheque forwarded together with the registration form. We regret that we are unable to accept Personal or Company cheques from outside Sweden.
3. Bank Account, SEB, Stockholm, No 5267-10 066 16, SWIFT-address ESSESESS. A copy of the transaction should be enclosed with the registration form.
4. For Swedish participants only: Postgiro: 65 37 38-5, Bankgiro: 644-8773. Please enclose a copy of the transaction with the registration form.
